Tax Checklist
- All W2's received from all employers
- All 1099-R (IRA & pension distributions)
- All 1099-MISC (non-employee compensation, rents, and other income)
- Any 1098-T/College Expense Statements
- Any 1098-E/Student Loan Interest Statements
- Social Security Benefit Statements
- Form 1095-A from Healthcare Marketplace
- Form 1095-B/1095-C Health Insurance Documentation
- Interest and Dividend Statements
- Form 1099-B (capital gains)
- Any 1099-G (unemployment)
- All other statements of income (K1, 1099-Q, 1099-C, 1099-S, W2-G, etc.)
- Amount of quarterly estimated payments paid
- Form 1099-SA (health savings account distributions)
- Advance Child Tax Credit Letter 6419
- Economic Impact Payment Letter 6475
- A copy of any IRS correspondence you may have received

Additional Information to Include
- Property tax paid
- Auto excise tax
- Mortgage interest paid
- Charitable contributions (over $500 must provide receipts. Please note the value of the donation)
- Child care paid (provider name, address and tax ID is required)
- Indiana college 529 contributions
- Rent paid (name and address of landlord required)
- Traditional IRA contributions
For Returning Clients
If your bank has changed, we will need new direct deposit information.
** If you got married or have a new dependent, we will need additional information: Full name, social security number and DOB.
** If you moved to a different state, we will need additional information: date you moved to different state.
